Root Cause Analysis (RCA) is a logical process

for analysing the cause-and-effect relationships of an event or situation. It finds solutions to ensure that undesirable events do not reoccur, and can also examine successful events to aid understanding of how the relevant scenarios can be replicated.

 

RCA is increasingly recognised as key to professional discipline and high industry sector standards, and its application is viewed as an indicator of best practice.
